   $50 for 1st applicant, $25 for each applicant thereafter. The application fee can be mailed or dropped off to our office:

2. Copy of photo ID

3. Proof of Social Security #
   ex. SS Card, W2, or Passport

4. Proof of income
   Two most recent pay stubs, for those self-employed 2 most recent tax returns
